INSURANCE: A Certificate of Liability Insurance for all organizations participating in Open Streets, due by OCTOBER 7, 2022. Certificates must name Bike Santa Cruz County as additional insured; address: PO Box 5485, SC, 95063-5485.
LIABILITY WAIVER: A Waiver of Liability TO BE SIGNED BY PARTICIPANTS is required if you will be HOSTING an activity of a physical nature (e.g. bicycling, dance, martial arts. A copy of your waiver must be submitted by *OCTOBER 7, 2022*. We can assist in providing a template.
SET-UP: You must provide all the equipment, staff, and structures you need to host your booth/activities, including tables, tents, and chairs. ALL PARTICIPATING organizations must provide their own small refuse and recycling receptacles.
